Permission forms for the merit badge college can be completed online here, or you can complete the paper forms and bring them to the merit badge college. Your signature on this page indicates your understanding of the included permission/consent forms. Only one Scout per form is allowed; you must complete a form for each Scout participating in the merit badge college.
